Switching from phenobarbital to levetiracetam is not the standard approach for poorly controlled canine epilepsy — the evidence supports adding levetiracetam as adjunctive therapy rather than replacing phenobarbital, and doing so requires a substantially higher levetiracetam dose than used in monotherapy. Phenobarbital induces levetiracetam clearance in a dose-dependent manner, making concurrent dosing pharmacokinetically complex. Phenobarbital co-administration increases levetiracetam clearance to 187 mL/h/kg compared to 71.8 mL/h/kg with bromide alone, resulting in significantly lower peak concentrations (37.5 μg/mL vs. 73.4 μg/mL at a normalized dose of 26.4 mg/kg) 4.
